Routledge agreed personal terms with Swansea on Monday and the 26-year-old will now undergo a medical on Tuesday before signing a three-year contract to seal his move for an undisclosed fee.
He will be joined at the Liberty Stadium by former Reading star Lita, who will also undergo a medical on Tuesday after a deal worth £1.75 million ($2.8 million) was reached with Middlesbrough last week.
Assuming there are no complications, both players could make their first Swansea appearances in Wednesday's friendly against Celtic.
Swans manager Brendan Rodgers has been working overtime to strengthen his squad ahead of the club's first season in the top-flight for 28 years and has already signed Danny Graham, Steven Caulker and Jose Moreira.
